This document provides additional instructions and guidance for institutions submitting an application to the Ontario Research Fund for small infrastructure funds (CFI’s Leaders Opportunity Fund).
Enter the name of the project, as provided in the application to CFI.
Enter the file number that has been provided by CFI for this project. Do not submit an application if CFI has not provided one.
Enter the full name of the institution submitting this application.
Enter the date that this application was finalized by your institution. It should be the same as (or shortly before) the date of signing.
Enter the name of the CFI program to which this project has been submitted: Leaders Opportunity Fund – Stream 1, 2, or 3.
Enter the street address (and floor or suite number) where the facilities will be located. A building name may be used if the address is not available. If the location is not known, enter "To be determined".
Enter the name of the city or town and the postal code in which the facilities will be located. If these are not known, enter "To be determined".
Enter the full name(s) of the main lead researcher(s) without title (e.g. John D. Smith). If there are more than three principal investigators, enter only one, and list the others in the Research Partners section.
Enter the name of the academic faculty or research institute in which the principal investigator will be situated for the purpose of this project.
Enter the name of the division or department of the faculty or institute in which the principal investigator will be situated for the purpose of this project. If several divisions or departments will share the project, enter "Shared".
Identify the total number of researchers who will be participating initially in the research activities for which the project is being proposed. These may include principal investigators, faculty members, research technicians, graduate research students and other participants at the institution or visiting from other institutions.
Enter in the format given the total estimated cost of the eligible infrastructure costs. This figure should be the same as the total cost that was reported in your application to the CFI.
Enter the capital funding that is expected from all sources other than the CFI or the Ontario Research Fund. This figure includes all funding that will be provided by the institution, other government agencies, private businesses, foundations, non-profit organizations, individuals, etc. Calculate and enter the percent of the total cost that will be provided by these other sources.
Enter the capital funding that is expected from the CFI, according to your application. Calculate and enter the percent of the total cost.
Enter the capital funding that is expected from the Ontario Research Fund. Calculate and enter the percent of the total cost. This percentage should not exceed 40% of the total cost of eligible infrastructure.
Enter in "Primary" the most appropriate Discipline Code from the listing of "Codes for Research by Discipline/Field of Activity" which is available on the Ministry web site. If the project also covers another discipline/field, enter that in "Secondary". Do NOT enter the discipline/field in words — just the code.
Enter in "Primary" the most appropriate Area of Application Code from the listing of "Codes by Area of Application" which is available on the Ministry web site. If the project also covers another area of application, enter that in "Secondary". Again, do NOT enter the area of application in words — just the code.
Identify, from the choices provided, the area that best describes the focus of the project. If none of the four main choices are appropriate, select "Other" and specify in the space provided the name of that sector.
Using simple, concise sentences, describe in no more than three lines the objective and key elements of this proposal. This summary description may be used, in whole or in part, in press releases or similar material. If more than three lines are entered in this box, the Ministry may edit it as required.
Describe, in no more than 250 words and in non-scientific terms, the major equipment to be purchased, the research to be carried out using the new equipment, and how the facility and the research might benefit Ontario.
Provide the full names of organizations that have agreed to provide funding for this project, other than CFI or ORF. If an acronym is used, provide the full name of the organization as well. Exclude any prospective funding partners who have not agreed in writing to provide financial or in-kind support.
Give names of individuals and institutions that will share in the use of this facility, and the nature of the collaborative work that will take place in the proposed infrastructure. Indicate if any research partner will be providing financial support.
In determining whether to approve the project for ORF funding support, the Ministry will consider the degree to which the proposal will address these specific evaluation criteria that are shared with CFI. Describe how your project will address the listed objectives, using as much space as necessary. This may be similar to the information you have submitted in the corresponding application to the CFI.
Provide the full name and title of the research director, manager or other administrator who is coordinating applications for research funding and who may be readily contacted for additional information. This person need not be the signing officer of the institution.
Provide the business telephone number of the Institution Contact, including extension number. Enter the email address and the full mailing address of the Institution Contact.
The application must be signed by the vice-president, research (or equivalent) of the lead institution.
